Speech Therapy Evaluations
New patients will need to complete an evaluation before beginning therapy services at Building Blocks Therapy Nook. This evaluation process allows us to get to know the patient and caregivers in order to develop a personalized, evidence-based plan of care to best serve each individual.
Evaluation sessions vary in length depending on the patient and needs being assessed. Upon completion of the evaluation, an evaluation report will be sent to you and your primary care physician. Recommendations for therapy will be provided in this evaluation report. Your evaluating speech-language pathologist will contact you to review the evaluation summary and explain treatment recommendations.
Scheduling an evaluation
Your evaluating therapist will send you information to create an account with Fusion’s Patient Portal where you can complete intake and registration paperwork.
A referral from the patient’s primary care physician is required. Your physician can fax the referral to Building Blocks Therapy Nook LLC.
Upon completion of the referral process and intake/registration paperwork, you will be contacted by a speech-language pathologist to discuss your needs and concerns and to schedule the evaluation session.
